Submit not disabled with acceptance activated

Hi,
my form has acceptance activated:
in dev site all works perfect (see https://goo.gl/GNlh29)
in production site not works (see https://goo.gl/N4MyEQ).

Consider that in production site the form until yesterday worked, today stop works.
And consider that I use polylang (I created two identical forms with the only difference of translated messages).

HELP…
